Staff Activity Report

Care Plan

The Care Plan type filter will include relevant information related to patient care plans in ChronicCareIQ including:

  • Patient Information:

    • Includes patient name, date of birth, and patient ID for identification purposes.

  • Care Plan Dates:

    • Records the care plan date, upload date/time, and patient viewed date/time for tracking.

  • Staff Involvement:

    • Documents the staff user responsible for building and uploading the care plan.

  • Provider and Case Manager Details:

    • Specifies the patient’s monitoring provider and the assigned case manager.


Patient Enrollment

  1. Time Period: This column specifies the duration or intervals for which the data is being reported. (e.g., weekly, monthly, quarterly). This filter can be run for a custom range of months and may be grouped weekly, bi-weekly, monthly, quarterly, bi-quarterly, yearly, and daily.

  2. Enrollments: Represents the total number of patients enrolled within the specified time frame.

  3. Enrollments via Campaign: Specifies the number of patients enrolled specifically through campaigns.

  4. Deactivations: Indicates the total number of patients who were deactivated or removed from the system during the reporting period.

  5. Deactivations via System: Represents the number of patients deactivated due to system-related actions or settings.

  6. Active Patients: This column shows the total count of patients who are currently active within the system.


Patient Enrollment by Staff

The Patient Enrollment by Staff type filter will show a summary of your staff and how many enrollments they were responsible for in the selected period in both line graph and tabular formats.


Patient Enrollment by Provider

Similar to the Patient Enrollment by Staff type filter, Patient Enrollments by Provider will show provider enrollments for the selected period of time in the same format.


Staff Utilization

The Staff Utilization type filter will show total logins and summarize other time spent in the software including chart time, dashboard time, offline time, total time, number of comments, and the volume of new patients.

  • Dashboard Time: Generated when staff views the list of patients on the dashboard. Time is limited on a per-patient basis to 10 seconds every hour, for a maximum of 30 seconds per day.

  • Offline Time: The amount of billable time from either the CallerIQ phone integration or comments with offline time.


Staff Comments

The Staff Comments type filter will display all chart comments for the selected date range. It can also be filtered by record type (comment or phone call) as well as billable activity (All, Billable, CCM, TCM, G Codes).

It will display the patient information, the billing code that the comment is contributing toward, the time stamp and duration for the comment, note text, and the staff information.

Monitoring Time

The Monitoring Time type filter is the best for tracking the total time spent in the software for the practice as a whole. It will summarize the total offline hours, online hours, and the total time for the selected date range. The date range may be grouped by day, week, month, quarter, and year.


Staff Logins

The Staff Logins type filter is used to track the total number of staff logins. It can be run for a range of months and may be grouped weekly, biweekly, monthly, quarterly, bi-yearly, yearly, or daily.


Staff Login Time

The Staff Login Time type filter is a good breakdown of the total logged-in time per staff member. It will also provide what percent of that total time was spent on patient charts.

  • Headers: Staff Name, Month, # of Logins, Logged in time, Chart time, Chart Percentage (Chart time divided by Logged in Time)

    • Can be run for today, the last 7 days, the last 30 days, this week, this month, last week, last month, Custom date range, or All Dates

    • may be grouped weekly, biweekly, monthly, quarterly, bi-yearly, yearly, or daily


Audit by Staff Details

The Audit by Staff – Details type filter is a full audit for any single user. It defaults to auditing the user that runs the report. If you wish to audit a different staff member check the “Select other staff member” check box on the filters page. It will pull the date and time for any given activity, the activity type, activity description, patient (if applicable), and the duration.

Can be run for today, the last 7 days, the last 30 days, this week, this month, last week, last month, Custom date range, and All Dates.


Audit by Staff Patient Summary

The Audit by Staff – Patient Summary type filter is simply a summary of the time any given staff member has spent on each patient.

It defaults to auditing the user that runs the report. If you wish to audit a different staff member check the “Select other staff member” check box on the filters page.
