Patient Information Report

The Patient Information Report is designed to offer comprehensive insights into the various facets of patient interactions with our system. From tracking utilization patterns to monitoring health records, these reports empower you to retrieve vital information that may be crucial for informed decision-making and enhancing patient care.

Patient Detail

The Patient Detail type filter will include important demographics and monitoring information for active patients.

This can be run in “Concise” or “Expanded” formats.

The Concise format of the report is designed for easy reading when browsing online:

The Expanded format will generate all of the fields in columns for the easy exporting of information. You may also choose to de-select certain fields:

Patient Name, Date of Birth, Race, Ethnicity, Gender, Patient ID, Monitoring Start date, Protocol, Status, Last Status Update, Workflow, Score, Score Delta(DAY), Score Delta(WEEK), Timer, Days Remaining (if monitoring is not perpetual), Care Plan, Care Plan Date, Contact, Billing Type, Email Address, Primary Phone, Home Phone, Mobile Phone, Secondary, Work phone, Last Phone Call, Call Preference, Zip Code, Primary Payer, Secondary Payer, Case Manager, Monitoring Provider, Schedule, Reminder Type, Consent Status


Patient Deactivations

The Patient Deactivations type filter will show all deactivated patients within the select time frame, the date they completed the program or were otherwise deactivated, and the reason why:


Answer Detail (Combined)

The Answer Detail (Combined) type filter is a large export of every patient response for the selected time frame. This report is also available as an outbound integration to your EHR. The integration will send the report for all answers for monitored patients for the previous calendar day, excluding any survey responses. It can be narrowed to device measurements only OR all answers.


Answer Detail (Expanded)

Similar to the Answer Detail (Combined) type filter, the expanded format is an export of every patient response for the selected time frame but it displays each question as a row instead of a column.

    • Headers: Last Name, First name, DOB, Pat ID, Protocol, Timestamp, Answer Numeric, Answer Formatted, Flagged, Question ID, Question Name, Measurement Type, Device, Entered By, Score, Alert


Patient Monitoring

The Patient Monitoring type filter is a summary of all patients’ vitals monitored during the selected time frame, including blood pressure, blood sugar, weight, temperature, heart rate, oxygen saturation, and peak air flow.

It will also include other relevant information such as patient monitoring start date, end date, completed reason (if applicable), and total monitoring periods completed and skipped.


Time Quantification

The Time Quantification type filter is a breakdown of all billable time accrued for patients in the selected month. You can see total billable time, and which actions contributed to that time (Offline time added via comments, CallerIQ phone time added through a phone integration, dashboard review time, or online (chart time).


Patient Schedule

The Patient Schedule type filter is an overview of all of your active patients in the selected month, their protocol, their contact type (Phone or Technology), their schedule (Daily, Biweekly Monday, MWF, etc), and their 4 latest monitoring periods (most recent and next 3 scheduled).


Clinical Comment

The Clinical Comment type filter will pull all comments posted to patient charts in the selected time frame. You can also filter patient messages received.


Patient Enrollment

The Patient Enrollment type filter will show all newly activated patients within the selected time frame, the date they were enrolled, who enrolled them, and their consent status and date.


Monitoring Alerts

The Monitoring Alert type filter presents historical data on patient warnings and alerts based on vital readings. This is a good filter to view overall trends in your patient population to assess the effectiveness of your chronic care management program.

This can be filtered to Phone or Technology patients, as well as what measurement type you want to view.


Patient Monitoring Goals

The Patient Monitoring Goals type filter enables care teams to easily track individual patients’ goals and monitor their progress within a comprehensive, centralized view. This report can be filtered by Monitoring Goal.
