Navigation Training – Patient Chart
The Patient Chart Navigation training video explains how to utilize the patient chart section of ChronicCareIQ. Here, users will learn detailed information regarding patient information, response data, patient graphs, chart tabs, and much more.
Patient Header UI
The Patient Header UI more closely resembles an EHR/EMR chart and docks the patient information and chart options at the top of the page.
The header “docks” the chart functions at the top of the webpage so the user can have them available without having to scroll within the chart. The header will always display the patient’s name, date of birth, and EMR ID (if entered into the patient demographics field) at all times. The patient information and action options will appear in the purple header:
The options for actions in the chart are the same choices that were previously displayed above the graphs. This location allows the user to have the options available at all times, regardless of scrolling down the page.
The ability to dock the comment dialog remains with this new chart header. Staff can use the dock comments icon to park the comment at the top of the page. This allows the user to have the comments open while navigating elsewhere in the chart at the same time:
The full comments dialog to add/subtract offline time, post date comments, etc. can still be used by clicking the dock icon to return it to the full dialog popover.
The ability to access some of the top dropdown tabs from the patient chart is also available.
Monitoring Technology Patients

Patient Statuses and Actions
Alert
and Warning
Prioritize patients in these statuses as they indicate clinical thresholds have been exceeded.
Use the pie chart on the dashboard to filter and review these patients.
After review, mark the patient as “Reviewed,” signified by a blue check mark
Direct Patient Messages – Alert Status
Review patients in “OK” status from their chart or dashboard list.
These patients report no issues and are within clinical boundaries.
OK Status
Review patients in “OK” status from their chart or dashboard list.
These patients report no issues and are within clinical boundaries.
Awaiting Answer
or Late Status
Indicates patients are current but awaiting responses.
After two missed answering periods, the status converts to the grey “?” indicating lateness.
Reach out to ensure compliance.
Pause/Resume Monitoring
Used for patients temporarily unavailable (e.g., vacation or hospitalization).
Access through the “Actions” menu to pause reminders and monitoring.
Resume monitoring via the same menu when the pause period ends.
No Login Status
Indicates a patient hasn’t completed their account setup.
Staff should assist with enrollment to ensure compliance.
Pink with a red email overlay signals an invalid email requiring updates.
Locked Status
This signifies another staff member is actively reviewing the patient chart.
Finished Status
For patients who’ve completed monitoring, decide whether to continue or deactivate.
To enable ongoing monitoring, update the protocol to “perpetual monitoring.”

Monitoring Phone Patients
Phone patients are patients who do not interact with ChronicCareIQ directly and do not completely set up a patient account.
Phone contact patients can be identified from the dashboard easily because a phone icon will appear with their contact number. A patient without this phone icon is a technology contact patient
Recommendations for Monitoring Phone Patients
Monitoring Schedule:
Set biweekly or monthly monitoring schedules for phone patients.
Dashboard Configuration:
Add the following columns to the dashboard for effective tracking:
Phone Number
Last Call
Call Preference
Save this as a “Favorite” view using the gear dropdown menu.
Identifying Patients Needing Calls:
Use the “Needs Review” filter to quickly identify phone patients requiring follow-up.
Status Updates:
During calls, manually enter patient responses in the Answers tab.
The entered answers will determine the patient’s clinical status, viewable through the Patient Status Icons.

Converting Patients to Technology
Converting phone patients to technology is easy! It also streamlines the care team’s workflow and makes tracking patients more efficient. Follow these simple instructions to perform the conversion:
Identify phone patients who have a smartphone or computer, but aren’t reporting data via technology
Navigate to the patient’s chart
Select the Edit Patient tab
Click the phone icon next to the patient’s phone number to open the contact preference
Click the Technology option
Click the blue Modify Patient button at the bottom of the screen to complete
Select the Actions tab & choose Send Communications from the list of options
From this send communication dialog box, you can send the patient a “Invite/Set Up” via email, text to set up in a browser, or text with mobile App link to download:
Patients can click the link to go directly to the Apple Store or Google Play to download the CCIQ app as well. Patient instructions are available here. Provide the patient with their username (typically their email address) and password (8-digit date of birth).
Patients who prefer to get reminders via email or text message can simply sign in via their web browser by clicking the link in the welcome email. Patient instructions for web use are available here.
Pause Patients
There are a variety of reasons a patient may want to temporarily pause their health status update reminders. This is most commonly requested when a patient has been admitted to the hospital or is on vacation and is unable to enter their health updates.
ChronicCareIQ offers an easy way to pause reminders. Follow the steps below to pause monitoring:
Navigate to the Patient Chart
Select the Actions tab
Choose Pause Monitoring from the drop-down menu
Click the option to Pause Monitoring
Enter the applicable dates to pause reminder emails, texts, or mobile notifications
Please Note: The system will set a default start date of “today” and an end date of “seven days from today”
Click the blue Pause Monitoring button at the bottom of the dialog to complete

Billing Types
In the patient’s chart, where their demographics are displayed, you can enable or disable different billing types by clicking the paper and pencil icon next to Billing Type.
In the Patient Billing Type window, you have two options:
Patient is Not Billable – The patient will not be billed for any codes, and no codes will drop in their chart.
Generate Billing Codes For Patient – The patient will be billable, and codes will be generated in their chart.
The Consent Status drop-down indicates whether the patient has given consent for billing. Options include:
Consent Declined
Consent Given
Consent Needed
Externally Documented
Not Documented (Phone Patient)
If the patient is eligible for billing, you will see the four columns:
Type – Billing type enabled for your CCIQ Account (e.g., RPM, RPM Education, CPM, BHI Base, CCM, PCM, ACPM).
State – Indicates billing eligibility (Enabled, Disabled For Patient, Requires Education (RPM Education), Protocol Not Eligible).
Action – Displays the next step required (Enabled, Disabled, Requires Patient Education (RPM Education), To Enable, Change Patient’s Protocol).
Consent Billing Type – Indicates whether the patient has consented to billing (Yes or No).
Customized Billing Settings
ChronicCareIQ offers customizable billing settings at account, patient, and monthly levels, providing robust control over billing codes. These settings allow practices to manage billing efficiently based on specific requirements.
Account-Level Settings:
Authorized by Admin Users via CCIQ support.
For global billing code adjustments, contact [email protected].
Patient-Level Settings:
Patients are defaulted to trigger any eligible billing codes based on requirements.
Specific billing parameters can be set through the “More” tab in the patient chart.
Examples:
Enable or disable individual billing codes (e.g., CCM, RPM).
Changes are applied immediately and logged in the audit system.
Maximum Unit Settings:
Apply limits based on local MAC or CMS carrier judgment (e.g., limit the number of complex CCM codes like 99489).
Specific Provider Settings:
Assign billing responsibility for specific codes to designated providers (e.g., RPM device codes billed under a secondary provider).
Met Date Type Settings:
Customize billing cycles for specific patients (e.g., bill RPM codes on the 28th of each month to meet insurance requirements).
FQHC/RHC Billing:
Clinics with FQHC/RHC designations can configure billing for eligible codes. Contact CCIQ support for assistance.
Turn Off Billing During a Month:
Temporarily suspend patient billing for the rest of the month.

Reminder Type
Reminder Type: Configurable setting that allows clients or patients to adjust the scheduled alert time. By default, reminders are sent at a preset time (e.g., 8:00 AM). Users can override this setting and specify an alternate delivery time (e.g., 10:00 AM) for any scheduled day.
This option provides flexibility in aligning reminder delivery with individual user preferences and daily routines.
Click on the “paper and pencil” icon to open up the Reminder Type settings
In the Reminders drop-down, select “Enabled”
In the Reminder Time drop-down, input the time
In the Reminder Type drop-down, select “Email” or “Text”
Once finished, click on the blue “Modify” button
