Digital patient consent is available within CCIQ when the patient sets up and logs into their ChronicCareIQ account. When digital patient consent is set up, patients will be prompted with the practice’s electronic consent form upon their initial sign-in (see image example below).

NOTE: When a new specific billing type is added to a patient’s chart, the patient will be prompted to consent to it upon their next login. Until consent is provided, the billing type will appear crossed out and remain non-billable.

Customized Consent Form
The client Admin can provide practice program consent language as a customized consent to ChronicCareIQ or use the default consent provided. Go to the Patients tab, then Consent Form:

View Consent Status
Once consent is given or declined by the patient via technology login, CCIQ will document it within the patient’s audit log. Staff users also see a message alert in the Task panel for the patients who decline consent. This allows staff to take appropriate action for these patients, i.e., deactivation or outreach.
Patients will have the option to accept or decline consent. The consent status is available for users within the Billing Type section of the patient chart and for patients who are pending or have declined consent by clicking the patient billing type on the CCIQ Monitoring Dashboard.
This information is also available within the patient’s audit log. Any patient who does NOT have consent marked as complete in ChronicCareIQ will have a warning on the dashboard and in the patient chart:

Pending Program Consent
Patients who decline consent will be “auto-disabled”. When enabled by CCIQ Support for an account, patients are fully blocked from account access of the CCIQ portal.

Staff can enable the patient and request another consent to be done electronically
Staff will get a decline consent notification on the patient’s chart. Patient will NOT be billable until consent is acquired.
Manual Consent Entry
NOTE: Staff users can manually update the patient’s consent status if needed by accessing the consent drop-down menu. To perform the manual update, follow these steps:
Navigate to the patient’s chart
Click the Billing Type edit button
Access the drop-down menu in the consent section to choose the appropriate options:
Consent Given, Consent Declined, Consent Needed, Externally Documented, and Not-documented (phone patient).
