Patient Audit Log and Export

To access and download a patient audit log, simply navigate to the patient chart. Once in the chart, the audit log is viewed under the “Audit Log” tab or can be viewed using the “Export” tab.

The audit log shows several items to the staff user:

  • The total time accrued for the patient (updated every 16 minutes)

  • The billing codes the patient is eligible for during the month selected

  • Device data count (if the patient has a connected device)

  • Future monitoring periods the patient is expected to answer protocol questions

  • Complete list of all actionable entries in the patient chart and associated times (see truncated example below):

This audit log is designed to show exactly what time is accrued on the patient and the history of actions taken in the patient chart by staff. Patient data entries are logged as well but note that patient time does NOT count for billing purposes, only staff time.

Additional information about an action logged in the patient audit log can be seen by clicking on the “Activity” logged. In the example patient, the staff member made an offline comment with call notes on 12/15/2023 at 2:14 PM. Export Patient Audit Log

To export the patient audit log for printing, external saving, or providing for audit reasons, click the “Export Month” or the “Export Custom Dates” when on the Audit Log tab in the patient chart. The Audit Log PDF is the document of proof of time accrual and tracking actions within ChronicCareIQ.

  • Export Month – Will automatically export the full Audit Log for the selected month.

  • Export Custom Dates – this will bring up a dialog for the user to choose the parameters and type of export to be viewed or downloaded.

The Audit Report can also be viewed or exported using the “Export” tab on the far right side of the chart tab options. This is simply another way to directly export patient data for auditing purposes.

Specific Requirements Missing

The dashboard icons will direct the staff to the patient audit log if specific requirements are missing for the patient to qualify for billing codes they may be eligible for. The Audit Log “Pending Billing Codes” panel explains in detail what is needed to qualify for a code.

For example, this patient has a “specific requirements” yellow warning on the billing icon:

Navigating to the patient “Audit Log” tab will show the staff member that this RPM patient is missing the required interactive communication to qualify for the code. Once the patient has a call logged or interactive comment designation, this patient will drop the base RPM code 99457:

Once a patient has exhausted all time-based billing codes they are eligible for (based on billing type and account/patient billing settings), a green banner will appear on the patient audit log. The patient billing icon will also receive a blue check mark so staff can easily identify patients who have completed billing for the month:
