Sending Communication Action
Staff members can send new patient invites or re-send patient protocol questionnaires using the “Communication” Panel in ChronicCareIQ.
The Send Communication dialog box will open:
Using the “Send Communication” Panel, staff can generate a variety of communications to the patient:
Send invite/set up to patients who have NOT logged into ChronicCareIQ
Send reminders via email (option to save email to patient chart)
Send reminder via text (option to save mobile number to patient chart)
Send reminders via mobile App notification (only for patients with mobile App installed)
Sending an Invite
To send an invite to patients who have never signed into ChronicCare IQ, click on the “Communication Reason” dropdown and select “Invite Setup Account.”
Sending an Answer Reminder
To send an answer reminder to a patient, select “Answer Reminder” from the “Communication Reason” dropdown.
A box will appear so you can enter either an email address or a phone number for the patient.
System Communications History
All communications sent to patients via ChronicCareIQ are logged on the “Communications History” page within the patient chart. This page gives staff insight into the types of communications sent to patients and tracking of those communications if available. To access this page, select “Coms History”:
Communications History will show system and manually generated emails, texts, and mobile App notifications sent to patients. Click on the icon beside the type of notice to see tracking of email or text delivery, clicks, and opens.
PLEASE NOTE: not all emails and mobile phone carriers allow tracking, so data might not be complete. Mobile App notifications are NOT trackable due to manufacturer and App Store rules, so no tracking will be available for mobile App push notifications.
Communications History will track the following communications sent to a patient:
Invite/Set Up Account
Answer Reminders
Password Reset links
Message From Care Team
Other: Identified communications sent to patient
Clinical Alert (when enabled to send to care team)
