Campaigns are a feature of ChronicCareIQ that enables a practice to send digital announcements (in bulk) to all patients or to select patients who are identified as eligible for CCM, RPM, and/or PCM services. The campaign will send the patient a message via email (if they have one on file) OR an SMS (text) if they do not have an email on file.
NOTE: Only ChronicCareIQ Admin Users can create campaigns. If you are not an Admin user, you will not have these functions available to you
There are two types of campaigns: enrollment campaigns and announcement campaigns. Once a campaign is created, a CCIQ support team member will review it and approve the draft before it is sent out (turnaround time is typically no later than 1 day). See the descriptions below of each type.
Announcement Campaigns
Patients who receive announcement campaign messages DO NOT have the opportunity to opt in and create an account in CCIQ. Announcement campaign messages are used to communicate to patients about any information the staff deems appropriate. Such examples include information about office hours for the weekend/holidays, the status of the office/practice, reminders to enter status, etc.

Enrollment Campaigns
Enrollment campaigns are designed to increase the number of actively monitored patients in your care management program.
Patients who receive enrollment campaign messages have the opportunity to opt in and create an account in CCIQ as a technology patient (patients who will provide their health status using a computer or smartphone).
The message will include a link for the patients to click on to enroll, which will provide a step-by-step guide through the process of creating an account outlined in the Patient Login Instructions video.
How to Add Campaigns
The staff user account must have Admin permissions to add campaigns. Click on the Patients menu tab and select Campaigns. You will see the options to add enrollment and announcement campaigns:
Steps to Add an Enrollment Campaign
Name – is an internal field that only staff will see. It does not show to patients and is the title displayed to staff for historical sent tracking.
Subject – is the subject title of the email that the patients will receive
Reply To – is the email address that patients would reply to if they need to (creating a general care management email for your practice is recommended)
If you don’t want patients to reply, you can create a fake/spoof email and add it
Protocol – select a default protocol to use for patients who enroll. If sending a mass campaign to all patients, then we recommend using a very generic protocol or having a custom protocol created for this purpose. Example generic or custom protocol to use: Health Maintenance or “Enrollment Protocol. Once the patient enrolls, the protocol can be changed for monitoring.
Provider – select a default provider that will be set for patients who enroll. If sending a mass campaign to all patients, then we recommend using the primary supervising provider or creating a provider user for this purpose. Example generic provider user to create: Smith Medical, MD, or Enrollment Provider. The actual monitoring provider can be changed once enrollment is complete.
Case Manager – select a default case manager who will be set for patients who enroll. If sending a mass campaign to all patients, then we recommend using the main case manager or creating a clinical user for this purpose. Example generic clinical user to create: Case Manager 1 or Enrollment Case Manager. Once the patient completes enrollment, update the case manager’s name to the appropriate monitoring case manager.
Message – type in a custom message to patients. Web links can be included in the message. CCIQ provides default templates for CCM and RPM enrollment that can be used. under the Message Template dropdown. These templates can be edited or customized to practice specifics, diagnosis specifics, or other care management types, such as PCM, BHI, etc.
On the scheduling tab, set a custom send time and send frequency (see next section)
Scheduling of Enrollment Campaigns
ChronicCareIQ enrollment campaigns can be scheduled to be sent in customizable batches. The staff may wish to send all invites at one time or engage in a Drip Campaign where a specific number of patients are contacted per day or week. Clients can specify the send date/time, the frequency, and/or the total number of patients to be invited per send:
For example, a practice has identified 300 patients to enroll in their CCM/RPM program. The settings are to start the campaign today and invite 100 patients per weekday. With this setup, it will take 3 business days to invite all patients flagged for the campaign:
NOTE: once a patient self-enrolls in CCIQ, the protocol, monitoring physician, and specific case manager can be changed within the patient chart if needed.
Logo URL Instructions
To add a Logo to the enrollment or announcement, use a URL link to the desired logo. This is not a file upload, but a link box to a URL image of the logo on the internet.

How to Flag Patients for an Enrollment Campaign
Patients can be flagged for Enrollment Campaigns in several ways. Once the campaign is created, clients can provide the list of patients to receive enrollment using the following methods described below.
NOTE: To receive a campaign, a patient must have a correct email address or mobile cell phone number in their unclaimed account. Without at least one of these pieces of information, the patient will be unable to receive an invite to join your CCM/RPM program.
Use the ChronicCareIQ Campaign Export
ChronicCareIQ has a function to identify and export a list of suggested patients to enroll in CCM services. ChronicCareIQ support staff can pull this list of suggested patients for clients with an initial patient import or demographics EHR integration.
NOTE: the patient imports must contain diagnosis codes and ChronicCareIQ recommends a basic payor description column (such as “Medicare or Medicare Advantage”
The criteria used to search and generate the list of potentially eligible patients are:
Unclaimed patient
Not dead (no deceased date)
TWO diagnosis codes that are labeled chronic
Primary payor included the word “Medicare” OR if the primary payor field is blank, then adds >65 years old. (this is how those under 65 yo appear on the list – they have a form of “Medicare” listed as a payor)
Use the Patient Search “Group Edit” Function to flag for Enrollment
Using the Patient Search tab, a group of patients can be chosen specifically by the client for enrollment. To select patients in this manner, go to the Patient Search page and use the Advanced Search function. In the Account Status filter drop-down, select Unclaimed and hit the blue Search button:
Expand the list to show the maximum number of patients (1000). If this list is too large, you can further refine it by adding specific ICD-10 codes. You can also limit the search to use only patients “65 and older” in the Age filter drop-down. Use the Group Edit button to choose the patients you wish to include in the enrollment and Flag those patients for Enrollment:
A confirmation screen of the flag for enrollment action will appear once this is completed.
Once you have appropriately flagged the desired patients, let your Customer Success Manager or ChronicCareIQ support know so they can add those patients and approve your campaign for sending.
Client Provided CSV list of patients – clients with EHR Integrations
For clients with EHR integrations including the patient demographics feed, clients can use the secure upload Any File Any Format function on the Patient Search tab to upload a CSV file of patients to be added to the enrollment campaign. Since the patients identified on the EHR-generated CSV are already loaded into ChronicCareIQ as unclaimed patients, this CSV file should include the following fields:
EMR Patient ID
Date Of Birth
Client Provided CSV list of patients – clients without EHR Integrations
Clients can use the secure upload Any File Any Format function on the Patient Search tab to upload a CSV file of patients to be added to the enrollment campaign. This CSV file can usually be pulled from the client EHR and should include the same fields as an initial patient import:
EMR Patient ID
Name: first and last are required. Middle name is optional.
Date Of Birth
Phone Number (mobile for SMS text invites)
Email Address
Diagnoses in ICD-10 code format
Campaign Reports
The Campaign Reports in CCIQ track the effectiveness of enrollment and announcement initiatives, offering valuable metrics to optimize engagement strategies. The main reports are:
Campaign Summary Report: This report summarizes key engagement metrics, such as:
Total patients added to the campaign
Emails sent, delivered, opened
Patients who started enrollment, marked as interested, or unsubscribed
Separate data for emails and SMS messages.
Campaign Detail Report: Focuses on patient interest, identifying patients who marked themselves as “interested” but did not fully enroll. Clinical users can follow up with these patients to discuss the program and update their status as “Not Interested” if needed.
Patient Enrollment Report: Displays the list of all patients who enrolled within a specified time period.
These reports provide actionable insights into campaign performance and patient engagement.
