Why Does CCIQ Require a Patient File Import?
Patient information is needed to streamline the patient enrollment process and ensure your team isn’t spending extra time manually entering patient data. This also assists in CCIQ reporting capabilities and call tracking.
How Do I Generate the File in my EMR or Practice Management System?
Every system’s reporting capabilities are different. If you are unsure how to generate a report containing the requested information, please get in touch with your CCIQ Implementation Specialist or your EMR or Practice Management vendor support team for assistance.
I am Unable to Generate a Report containing ALL of the Requested Information. What are my options?
It’s not uncommon to experience issues when trying to generate reports containing ALL of the requested information. Please note that CCIQ can process multiple reports and does NOT require all of the requested information to be on one comprehensive report. Our friendly team will be happy to assist you with generating these reports in your EMR or Practice Management system if assistance is needed.
Patient Data Requirements
Required:
EMR Patient ID
Name, first, and last are required. The middle name is optional.
Date Of Birth
Phone Number (home, mobile, or work)
Desired:
Email Address
Diagnosis, ICD-10 codes
Gender
Race
Ethnicity
Optional:
Provider (for CCM monitoring)
Case Manager (for CCM monitoring)
Primary Payor
Secondary Payor
File Format Requirement
CSV (comma-separated value) format is accepted. Here’s an example template to download:
How Do I Send the Patient File to CCIQ?
Step 1: Login to ChronicCareIQ – app.chroniccareiq.com
Step 2: Select the “Patients” tab from the navigation bar at the top of the screen
Step 3: Click the “Secure Upload” option
Step 4: Click “Patient Import – CCIQ File Format” type, then select applicable CSV files for import
Step 5: Click the “Upload” button to complete the import
Step 6: A confirmation screen that your file was sent to ChronicCareIQ support will appear:
Please Note: Only accounts with a specially built auto-parser will have the “My EMR’s File Format” choice
