Phone Contact Workflow

This video provides an overview for monitoring patients who have been setup with phone contact type in ChronicCareIQ. Phone patients are patients who do not interact with ChronicCareIQ directly and do not complete set up a patient account.

Phone contact patients can be identified from the dashboard easily because a phone icon will appear by their contact number. A patient without this phone icon is a technology contact patient:

Patients who do not interact with ChronicCareIQ by technology means can be monitored by staff via phone calls. These patients have their status questions updated by staff members using the “Answers” tab and do not receive status reminder communications via email, text, or App push notifications.

ChronicCareIQ recommends phone patients be set to biweekly or monthly monitoring schedules.

To effectively monitor phone patients, ChronicCareIQ suggests staff add the following columns to their dashboard and save this as a “Favorite” view: Use the gear dropdown to include the following:

  • Phone Number

  • Last Call

  • Call Preference

To quickly identify phone patients who need a call for the month, choose the “Needs Review” section to filter your dashboard:


It is important to note that these patients are not eligible for the 30 second per day dashboard review time unless they have a connected device. Staff can manually enter patient question answers during the patient interaction and those answers will give a status for the patient.
