Time is accrued in patient charts via several ways in ChronicCareIQ. These ways include:
Dashboard time for active technology/device patients
Time tracked within a chart for staff performing active review patient data and chart areas
Call time logged via the CallerIQ integration with the phone system
Offline time documentation manually added to patient chart
For the Offline Time added to a patient chart for qualifying care management activities, the staff uses the “Offline Time” feature of the Comments or Log Call action to add this time. This manually entered time and documentation of care activities performed for patient management can be done for any qualifying clinical care done outside of an office visit. Time can also be subtracted if necessary and/or post-dated for previous-day documentation.
Adding Offline Time
To add offline time (time that was not tracked automatically by using ChronicCareIQ for care management activities), use the “OFFLINE TIME” option in the Comment or Log Call tab. Document all offline care management activities performed for patient and check the “Offline Time” box to add manually entered time:
Subtract Offline Time
To subtract any time that should not have been added to the patient chart or to correct time accrual mistakes, use the “Offline Time” option to “Subtract Time” from the Comment or Log Call tab. ChronicCareIQ recommends a note of documentation for this action:
Post Date Comment
To post date a date/time for a comment, use the “Post Date Comment” option checkbox from the Comment or Log Call tab. This will post the date of your care activity documentation to the proper date/time it occurred.
PLEASE NOTE: Please be aware that if you post date a comment for a previous billing month, it could update/change the billing codes generated for that patient during that previous month.
