Reports Guide

ChronicCareIQ Reports can be accessed by clicking the “Reports” option on the navigation bar at the top of the screen.

At the top of the Reporting Menu you will see a summary dashboard with 6 tiles that display various status summaries.

At the bottom of the menu, the reports are separated in to 8 categories that together contain over 40 different reports with various filters and may be exported in HTML, CSV, or PDF formats. This page will discuss the reports within each group and the information that can be obtained by running them.

Summary Dashboard

Status Summary – Displays a count for patients within each status category (Alerting, Warning, OK, Late, No Login). Clicking any of the status categories will take you back to the dashboard and display the applicable patients.

Monitoring Summary – Displays a count for the patients within each monitoring category (Active, Deactivated, Unclaimed). Clicking any of the monitoring categories will take you to the Patient Search and display all applicable patients.

Recent activity – A ticker feed of the 7 most recent patient activities such as viewing their care plan or modifying their protocol answers.

Recent Billing – A ticker feed of the 7 most recent patients to become billable and which code they triggered.

Patient Issues Summary – Displays a count for the patients within each patient issue category (Locked Account, No Login, Bad Email (Undeliverable or Missing), Missing Care Plan, Missing Diagnosis Codes, and Consent Issues (Consent Needed or Declined). Clicking any of the patient issue categories will automatically pull the corresponding report.

Patient Starts per Week – Displays a line graph that updates with a data point each Sunday to track total patients starting the program, which may be useful in tracking enrollment initiatives.
